Flagler Schools Wellness Center

Flagler Schools Wellness Center is an addition to the Flagler County School District benefit suite, where you are able to seek primary and acute care services with a focus on prevention and wellness management at $0 cost to active and retired employees who are on a Flagler Schools Medical Plan, along with qualified dependents.

Monday 7:00 A.M. - 4:00 P.M.
Tuesday 10:00 A.M. - 7:00 P.M.
Wednesday 8:00 A.M. - 5:00 P.M.
Thursday 10:00 A.M. - 7:00 P.M.
Friday 8:00 A.M. - 5:00 P.M.

The Center is closed for 1 hour for lunch, daily from 1:00 -2:00, closed on Saturday and Sunday, closed on the holidays listed below:

New Year’s Day Presidents’ Day
Memorial Day Independence Day
Labor Day Thanksgiving and the Day after
Christmas (Eve) Christmas Day

The hours of operation are subject to change. If this occurs, changes that affect the established schedule of hours of operation will be communicated to patients.

Walk-in Appointments

In order to be respectful of patients who have scheduled appointments, please call ahead to schedule a same-day appointment. This helps alleviate additional wait times and deters any inconvenience to you. Flagler Schools Wellness Center will do its best to fit you in; however, if a same-day appointment is not currently available, the staff will work to get you in as quickly as possible.

Please take your photo ID and current Florida Blue Cross Blue Shield health insurance card with you to your visit.

Lab Work and Vaccinations

  • Administer shots/vaccinations
  • Order, conduct, interpret, and consult on routine diagnostic lab work
  • The following lab tests can be collected and processed in the Wellness Center:
    • Glucose, blood sugar
    • Hemoglobin A1C
    • Lipid panel
    • Mono
    • Oxygen saturation
    • Pregnancy
    • Rapid strep
    • Urinalysis
  • Additional labs can be collected at theWellness Center and sent out for results
  • Can complete lab draws for routine labs with orders from outside providers

Sheri Hahn

Sheri Hahn, MSN, ARNP, FNP-C

Sheri Hahn, is a Family Nurse Practitioner-C with a Master of Science in Nursing from Maryville University,St. Louis, Missouri, where she graduated with honors. She has dedicated her career to providing comprehensive healthcare to patients across all age groups? from pediatrics to geriatrics. Her focus has been on managing chronic conditions, including hypertension, cardiovascular disease, and diabetes. She is passionate about screening, prevention, and eliminating the stigma surrounding mental health.

In addition to her work with chronic disease management and mental health, she is also a strong advocate for patient education. Sheri?s goal is to ensure that patients fully understand their health conditions, treatment options, and the steps they can take to improve their overall well-being. She firmly believes that informed patients make better decisions, and takes pride in helping individuals take control of their health.

When Sheri is not working, she enjoys spending time with family and traveling with her husband and ?travel family?exploring new cultures. Traveling is something Sheri values for the perspective it brings, and it helps her recharge so she can continue providing the best care possible to her patients.
