Flagler Schools HSHT Transition ProgramFlagler Schools Transition Services and Programs are available to support Flagler County students with unique learning needs. "Transition" refers to a coordinated set of activities that help a student with a disability move from school to post-school life. The process occurs over a period of several years and involves planning, goal setting, instruction, services and activities designed to make that move successful. Flagler Schools Transition programs directly align with the Classroom to Careers initiative.

The Flagler Community Transition Team (FCTT) works with multidisciplinary agencies and community businesses to improve career opportunities for students with disabilities who are entering the workforce after high school. The FCTT participates annually in Disability Mentoring Day, a national mentoring program for young professionals with disabilities.

Flagler Schools Transition Programs

High School High Tech
: This program provides high school students with all types of disabilities the opportunity to explore jobs or postsecondary education leading to technology-related careers.

Project SEARCH: Located at the Grand Villa of Palm Coast, the primary objective of this program is to secure competitive employment for students with disabilities.

STREAM Structured Education Courses and Managed Care and Supports: STREAM courses are designed to support young people with disabilities who wish to defer their high school diploma, but require the managed care and specialized supports offered through their service school. Each series of on-campus courses helps students become as independent as possible. 

TRAIL Transitioning youth with disabilities into Real vocational opportunities through Action planning and Individualized Learning: The TRAIL transition program provides workforce skills and training to young people with disabilities between the ages of 18–22. Based on the unique needs and learning abilities of each student, the program exposes students to a variety of career choices, while enhancing the skills necessary to become successful, independent, and employed.

The information is to be reviewed with families and students at each annual Transition Individual Education Plan (IEP) meeting. Documentation of review is to be provided in the appropriate sections within the IEP. A copy of the guide may be provided, emailed home to families, or accessed on the Flagler Schools website. Please contact your school-based Local Education Agency (LEA) for more information on locating the document or the contents.
