The State of Florida requires that all students entering or attending the 7th through 12th grade must have a Tetanus Diphtheria Booster. The Tetanus Diphtheria Booster must be completed prior to entering the 7th grade. In the past, we have had to send students home at the beginning of 7th grade due to them not having this booster shot. We want to avoid this in future.

The Health Department will provide the following immunization clinics:

Where:    Indian Trails Middle school

Dates:      Monday and Tuesday, April 21st and 22nd

Time:        3:45- 6:30 P.M.

Where:     Buddy Taylor Middle School

Dates:      Wednesday and Thursday, April 23rd and 24th

Time:    2:30-6 P.M

To set up an appointment, please call Gaetana Carroll at 437-7350 ext 2219. Please be assured there is no charge for the immunization. When you come to the appointment, please bring your insurance card if you have one. If your insurance provider covers the administration of immunizations we will bill them with no out-of-pocket charge to you. If you have no insurance,there will still be no charge.

For  families who are unable to attend the clinic with their child, the following individuals may bring the child and sign for the child's immunization: stepparent, grandparent, adult brother or sister, an adult aunt or uncle, or a possessor of a statement of permission to consent for medical treatment, signed by the parent/legal guardian, and notarized.
